An IHSS Paperwork Checklist for California Families
A step-by-step checklist for Medi-Cal, SOC 295, SOC 873, the assessment, provider enrollment, and EVV, without promising timing.

Work the IHSS packet in order: confirm Medi-Cal, submit SOC 295, return SOC 873, complete the home assessment, read the notice, finish provider enrollment if you will give the care, then set up timesheets and EVV. Timing is local. A missing page is the usual stall, not a missing diagnosis stack.

Your county IHSS office and Medi-Cal decide when a file is complete, who is eligible, and which tasks are authorized. This checklist is the family side of the same path.

Use a checklist so the file stays ordinary
IHSS is a sequence of named pages and one visit in the home. A checklist keeps that sequence visible when the week is busy. It does not create a deadline. It does not promise when a social worker will call. Your county office can tell you what is typical locally. The families page is the narrative. This page is the boxes.
You gather. The county decides. Helping a parent apply and the first week guide explain why this order exists. Check a box only when the page is actually sent or the conversation actually happened.
Before the IHSS application
- Confirm your parent lives in California in a home they choose. A hospital, nursing facility, or licensed community care facility is not “own home.”
- Write the home address and the county that should hold the case.
- Check Medi-Cal status. If it is missing, start BenefitsCal or the county Medi-Cal office.
- Ask whether a share of cost applies.
- Talk with your parent about staying home, who the employer is, and who they want in the room. See talking with a parent.
- Ask how an adult child can be listed so the county can speak with you.
- Name one sibling to track forms if more than one person is helping.
Application and Health Care Certification
- Download or request SOC 295.
- Use the name that already appears on Medi-Cal when you can.
- Get the recipient signature the form asks for.
- Send SOC 295 to the county IHSS office using the method that office accepts.
- Keep a dated copy and a note of how you sent it.
- Take SOC 873 to a licensed health professional, or ask the county what alternative documentation they accept.
- Return SOC 873 to the county. Confirm it is on file with one call if you need to.
Incomplete packets sit. Extra labs do not replace a missing signature. Eligibility restates why Medi-Cal and SOC 873 sit under the case.
Assessment and notice
- Write a typical-day list: bathing, dressing, meals, mobility, household tasks. See preparing for the visit.
- Decide who will sit in if your parent wants company.
- At the visit, describe actual help. Do not inflate. Do not shrink. See what to say.
- Write the worker’s name and any extra page they asked for.
- When the Notice of Action arrives, read the authorized tasks before you plan a work week.

Provider enrollment, timesheets, and EVV
- Decide who will give the authorized care: you, another relative, a friend, or a Public Authority registry provider.
- Complete county orientation, provider forms including SOC 426 and the provider agreement, and a Department of Justice background check where required. See enrollment steps.
- If you live with your parent, ask about live-in self-certification (SOC 2298).
- Ask whether Electronic Visit Verification applies, and which method to use.
- Record only authorized time after enrollment is finished.
- Ask about the first provider payment rather than assuming a date or amount.

Keep the list next to the folder
A checklist without copies becomes another conversation. Date every send. Store case numbers on the folder. Tracking paperwork is the filing habit that makes this list useful a month later.
If a box is stuck, talk to the county office once with the case number and the missing page name. Name the form, the send date, and what you need them to confirm. If the stall is a hospital week, read after discharge so you do not treat the hospital as the home address. If you want a person to sit with the packet, ask for help. - Do I have to finish every box before I send SOC 295?
- No. Send the application when it is complete and signed. SOC 873 can travel on its own track. Provider enrollment can start when the provider decision is clear.
- Which medical records go with the checklist?
- SOC 873, or acceptable alternative documentation the county names. Keep hospital charts unless a worker asks for a specific page.
- Who should run the checklist if siblings are helping?
