Checklists

Print these before the hospital, the county, or the agency call

Updated September 16, 2026. Reviewed September 16, 2026 against public CDSS, DHCS, Medicare.gov, VA, and CareScout 2025 California tables. County wages, hours, and wait times still change. Official .gov pages decide.

Adult children lose days to missing pages and vague tours. These HTML checklists are meant to be printed or saved as PDF from your browser. They do not file IHSS, admit anyone to a facility, or hire an agency.

Timing, wages, and hour totals stay with the county and with providers. Official CDSS, DHCS, and Community Care Licensing pages remain the source for forms and licenses. Based on public CDSS, DHCS, Medicare.gov, VA, and California Department of Insurance pages retrieved 2026-09-16. County, Medi-Cal, Medicare, VA, and insurer staff decide what actually pays. Hourly rates, hour totals, waiver slots, and wait times are local and change.

See if IHSS may apply

A coordinator can walk through Medi-Cal, county forms, and the home visit with you. The county still decides eligibility and hours.

For example: starting an IHSS application, provider enrollment, or county forms. Keep medical records and diagnoses for the county packet.

Questions people ask

Are these official CDSS forms?
No. They are family checklists that point to official forms and offices. Download SOC pages from the CDSS forms catalog.
Can I save a PDF?
Use your browser’s print dialog and choose save as PDF. That is enough for a fridge copy.
Do I need every box?
Skip a box only if it is already done. A missing SOC 873 is a common stall. Guessing wages is never required.
