Based on public CDSS, DHCS, Medicare.gov, VA, and California Department of Insurance pages retrieved 2026-09-16. County, Medi-Cal, Medicare, VA, and insurer staff decide what actually pays. Hourly rates, hour totals, waiver slots, and wait times are local and change.

Direct answer

Generally no for long-term memory care

Medicare generally does not pay for long-term memory care. A memory-care apartment is usually private residential care with supervision. That sits with Medicare’s custodial long-term care rule. A limited skilled nursing facility stay is a different benefit and a different setting. California families typically look at private pay, long-term care insurance, VA programs, and in some cases Medi-Cal waiver options. IHSS may help when the person still lives in their own home.

Who decides

Medicare and the plan decide skilled claims. The memory-care community sets private-pay terms. County IHSS staff decide in-home hours if the person still lives at home. DHCS decides waiver programs.

What to do next

If the person is still at home, read the dementia care page and IHSS protective supervision education. If you are touring memory care, open the memory care payment map and the cost page.

Memory care is usually a residential product, not a Medicare skilled stay

Memory care units charge for a secured or specialized residential setting, meals, and supervision for people living with dementia. That monthly fee is not the Medicare skilled nursing facility benefit. Medicare.gov’s long-term care page is the reason the long-term answer is generally no.

A hospital social worker may still arrange a skilled nursing stay after a qualifying admission. That stay has its own day rules and skilled-need tests. When it ends, the memory-care tour is a new billing conversation. Keep those folders separate.

If the person can still live at home

IHSS can authorize personal care and, when the county finds it is needed because of a mental impairment or mental illness, protective supervision. That is an in-home program. It is not a memory-care placement. Read dementia and Alzheimer’s care at home before you assume a move is the only path.

Private home care can sit next to IHSS if the family hires a licensed organization for extra hours. Keep IHSS timesheets and private invoices distinct. The county wage is local. See the CDSS wage-rate page rather than a statewide rumor.

If you are comparing communities

Ask what happens when wandering, exits, or two-person assists increase the fee. Ask whether the site participates in any Medi-Cal waiver. Ask whether a long-term care policy, if you have one, covers this level of residential care. Then read how to pay for memory care.

Official pages over marketing language

Communities sometimes say “we take Medicare.” That often means they can bill Medicare for a separate skilled service or that residents keep Medicare for doctor visits. It rarely means Medicare pays the memory-care rent. Ask for the billing desk in writing.

Questions people ask

Does an Alzheimer’s diagnosis create a Medicare memory-care benefit?
No. Diagnosis alone does not create a standing Medicare payment for a memory-care apartment. Ask about skilled needs separately from residential care.
Can Medicare pay for a short rehab stay, then memory care?
A qualifying skilled nursing stay may be billed to Medicare under SNF rules. The later memory-care apartment is usually a new private-pay or other-payer conversation.
Can the person stay home with dementia instead?
Many families do, with IHSS, possible protective supervision, and sometimes private home care. The county assesses safety in that home. See the dementia condition page.
Where are memory care prices?
Use the California cost of memory care page for CareScout planning medians, then ask named communities. This site does not quote a community’s private fee.

See if IHSS may apply

A coordinator can walk through Medi-Cal, county forms, and the home visit with you. The county still decides eligibility and hours.

For example: starting an IHSS application, provider enrollment, or county forms. Keep medical records and diagnoses for the county packet.
