A diagnosis does not authorize hours. The social worker looks at what the person can safely do alone in this house. Medicare generally does not pay ongoing custodial dementia care. Stay-home care is a task list, not a diagnosis trophy

IHSS looks at bathing, dressing, meals, toileting, moving safely, and other listed categories. Dementia changes how those tasks fail: skipped showers, the stove left on, walking out the door. Write those moments down. The social worker still has to see them in this home. A clinic letter helps the Health Care Certification. It does not replace the visit.

It is not payment for sitting and chatting. It is not automatic for every Alzheimer’s diagnosis. The county looks at whether the person can understand danger, whether they wander, and whether other resources already cover that risk.
