Does Medicare Pay for Assisted Living?

Updated September 16, 2026. Reviewed September 16, 2026 against public CDSS, DHCS, Medicare.gov, VA, and CareScout 2025 California tables. County wages, hours, and wait times still change. Official .gov pages decide.

Generally no. Medicare does not pay assisted living room, board, or custodial help. See private pay, long-term care insurance, VA, and California’s Assisted Living Waiver instead.

Based on public CDSS, DHCS, Medicare.gov, VA, and California Department of Insurance pages retrieved 2026-09-16. County, Medi-Cal, Medicare, VA, and insurer staff decide what actually pays. Hourly rates, hour totals, waiver slots, and wait times are local and change.

Direct answer

Generally no for room, board, and custodial help

Medicare does not pay the monthly assisted living bill for room, board, and custodial help. Assisted living is a residential setting. Medicare’s public pages cover limited skilled home health and limited skilled nursing facility care, not a standing assisted living rent. Families in California usually look at private pay, long-term care insurance, VA benefits, or Medi-Cal waiver options such as the Assisted Living Waiver.

Who decides

The assisted living community sets private-pay terms. Medicare and the plan decide skilled claims. DHCS and the waiver agency decide Assisted Living Waiver enrollment. This site does not admit anyone to a community.

What to do next

Ask the community what is included in the private-pay fee, then read the Assisted Living Waiver page if Medi-Cal may apply, and the cost of assisted living page for planning medians.

Assisted living is housing plus help, not a Medicare SNF stay

Assisted living communities charge a monthly fee for an apartment or room, meals, and help with daily activities. That package is custodial and residential. Medicare.gov’s long-term care page is the reason the answer is generally no. A limited skilled nursing facility benefit, described on Medicare’s SNF page, is a different setting with a qualifying hospital stay and skilled-need rules.

If a salesperson says “Medicare will pick this up,” ask them to show the Medicare.gov page that names assisted living. Then call the plan. Prefer documents over a tour script.

What California families actually mix

  • Private pay, using savings, a home sale, or family contributions
  • A long-term care insurance policy, if the contract covers residential care
  • VA Aid and Attendance, if VA finds the person eligible
  • Medi-Cal Assisted Living Waiver or other DHCS home-and-community-based options, when a site participates and a slot exists

Cost of assisted living uses CareScout 2025 California medians as a planning range. It is not a quote from a named community. Ask each community for the current fee and what happens when care needs rise.

IHSS usually stays tied to a person’s own home

IHSS is for eligible people living at home or in an abode of their own choosing. CDSS says an acute-care hospital, a long-term care facility, and a licensed community care facility are not “own home.” Many assisted living settings are licensed community care facilities. Do not move a parent and assume the IHSS hours move with the furniture. Ask the county before the move.

If the person could still live at home

Some families tour assisted living because they have not yet filed IHSS. If the parent wants to stay in their own home, read does Medi-Cal pay for IHSS and helping a parent apply before you sign a community contract. Compare care options puts the settings next to each other without a sales ranking.

Questions people ask

Will Medicare pay if the assisted living has a nurse on site?
A nurse on staff does not turn the monthly community fee into a Medicare skilled nursing facility stay. Those are different benefits and different settings.
Can Medicare still pay for doctor visits or hospital care while someone lives in assisted living?
Health coverage for medical care can continue. That is not the same as paying the assisted living rent. Ask the plan about medical claims, and the community about the housing bill.
Is assisted living “own home” for IHSS?
CDSS treats licensed community care facilities as not “own home” for IHSS. Do not assume IHSS follows someone into assisted living. Ask the county and the community.
What is the Assisted Living Waiver?
A California Medi-Cal home-and-community-based option that may cover some assisted living services for eligible people in participating sites. Slots are limited. DHCS decides.

See if IHSS may apply

A coordinator can walk through Medi-Cal, county forms, and the home visit with you. The county still decides eligibility and hours.

For example: starting an IHSS application, provider enrollment, or county forms. Keep medical records and diagnoses for the county packet.
