Dementia care at home, filed in the county where your parent lives

Reviewed September 24, 2026. Checked against public CDSS IHSS pages, the CDSS county IHSS office list, DHCS Medi-Cal, and California Department of Aging Caregiver Resource Centers. County wages, hours, and wait times still change. Official .gov pages decide.

Families search for dementia care at home in Los Angeles, Orange, San Diego, Santa Clara, Alameda, Sacramento, Riverside, San Bernardino, Contra Costa, and San Francisco. IHSS is still a statewide Medi-Cal program. The county where the person lives takes the file. These pages explain that geography. They do not quote wages, hours, or wait times.

Short answer

Dementia care at home in these counties means IHSS personal care, and protective supervision only when that county finds the person is at risk of injury if left unsupervised. A diagnosis of dementia or Alzheimer's is not an hour award. Each county page covers filing geography, neighbor-county mistakes, and the adult-child commute trap.

Who decides

Clinicians diagnose and treat. The county IHSS office authorizes tasks, including any protective supervision. Medi-Cal staff decide coverage. California Home Help explains the paperwork. It does not decide the case.

What to do next

Open the county where your parent lives, then the CDSS county IHSS office list. If the county you need is not in this set, use the statewide dementia page and that county's IHSS page. Other counties run the same program.

Questions people ask

Does a dementia diagnosis guarantee IHSS hours?
No. The county assesses tasks and unsupervised risk in this home. Two people with the same diagnosis can receive different notices.
Why are only some counties listed?
These ten are high-search filing counties. Every California county still runs IHSS under CDSS rules. Use the county IHSS page for a county that is not in this set.
Where does protective supervision fit?
It is an IHSS category the county may authorize when a mental impairment or mental illness creates a risk of injury if the person is left unsupervised. It is not automatic with Alzheimer's.
Can California Home Help decide the case?
No. County IHSS and Medi-Cal decide. California Home Help can help organize the packet. Sevah's home care team is an optional later conversation about in-home help.
