Why IHSS Is Not Around-the-Clock Care

In-Home Supportive Services pays an enrolled provider for tasks a county social worker authorizes so an eligible person can stay in their own home. It is not around-the-clock care and it is not a staffing agency for every hour someone is awake or asleep. Family presence overnight can be real and still sit outside the authorized list. Protective supervision, when authorized, is still a listed category with rules, not a synonym for around-the-clock coverage.

The county IHSS social worker authorizes specific tasks and hours after the assessment. Medi-Cal still has to be in place. The notice is the list that counts.

Authorized tasks are the product, not a live-in shift
Common questions families ask after a fall or a hospital stay include how to get around-the-clock IHSS. The program is built from listed services in WIC 12300 and MPP 30-757. A social worker ranks what the person can safely do alone and authorizes time for those tasks. What personal care covers is one bucket. Household related tasks are another. Accompaniment, paramedical services, and protective supervision each have their own rules.
That design is why IHSS can be essential and still not look like a facility staffing pattern. The county writes a notice, not a round-the-clock roster. Caring for a grandparent after a fall is a situation where this mismatch shows up often.
Family presence overnight is a different fact
Many households have a relative who sleeps in the home because they are afraid to leave. That presence can be loving and still not be an IHSS task. Companionship and watching the clock until morning are usually family arrangements or other benefits. Authorized tasks versus companionship keeps social sitting off the timesheet.
If nighttime help is actual hands-on work (toileting, repositioning, a transfer), describe those tasks at the assessment. Describe them as tasks that happen, not as a request to fill every hour of darkness.

Questions people ask
- If someone must be in the house all night, will IHSS cover that?
- Family presence overnight is not automatically IHSS. The worker authorizes listed tasks, and may consider protective supervision only when mental impairment or mental illness rules are met. Ask the county. Do not treat overnight sitting as a default product.
- Can we add up personal care until it fills the whole day?

- Is live-in the same as around-the-clock pay?
- No. Living with the recipient can change EVV steps on SOC 2298. It does not, by itself, authorize around-the-clock hours.
- What if the notice is smaller than the help we give?
