IHSS Versus Home Health in California

Short answer
IHSS is Medi-Cal personal and household help for someone living at home. Skilled home health is a different benefit for nursing, therapy, and related medical services ordered by a clinician. A family may use both. Neither one replaces the other, and neither one pays for a nursing facility stay.

IHSS vs home health in California
Families often use “home care,” “home health,” and “IHSS” as if they were one program. In California they are not. In-Home Supportive Services is Medi-Cal help with daily living so an eligible person can stay in their own home. Home health is skilled care, usually nursing or therapy, ordered by a licensed clinician and delivered by an agency for a medical need.
The split matters because the paperwork, the payer, and the person who comes to the house are different. Mixing the two in one conversation with a county worker can make it sound like you are asking IHSS to authorize skilled nursing, or asking a home health nurse to become the long-term helper. What IHSS pays for at home
IHSS can pay an enrolled individual provider for authorized personal care and related household tasks: bathing, dressing, meals, laundry, shopping, help moving safely at home, and other categories the county assesses. Protective supervision and paramedical services exist as separate IHSS categories when the rules and the notice support them. The Notice of Action is the list that counts.
The recipient generally hires that provider. Adult children are commonly allowed after enrollment. The State issues the check. Companionship by itself is not the design. Skilled procedures that licensed staff must perform are usually outside IHSS. Ask the social worker how a borderline task is treated rather than assuming it belongs on the timesheet.
