Staying Home with Medi-Cal IHSS in California

How IHSS supports staying home in California
The purpose of In-Home Supportive Services is in-home help for aged, blind, or disabled people who cannot safely remain home without it. That is a staying-home program, not a facility program. Medi-Cal is the coverage underneath. The county assesses which daily tasks need another person. The State can then pay an enrolled provider, often an adult child, for the authorized work.
Staying home still takes a real plan: who bathes, who cooks, who watches a stove, who gets to the clinic. IHSS can pay for parts of that plan when the notice authorizes them. It does not replace every neighbor, every family visit, or every skilled medical service. What IHSS is and IHSS versus home health keep those edges clear.
What own home means for IHSS
CDSS publishes living-arrangement language that families should take literally. The person must live at home or in an abode of their own choosing. An acute-care hospital, a long-term care facility, and a licensed community care facility are not own home. Those settings use other Medi-Cal or Medicare benefits. IHSS and nursing facility care is the companion page for that split.
Own home is not limited to a house the person owns. A rented flat, a senior apartment that is not a licensed care facility, or living with an adult child in that child’s house can be an abode of one’s choosing. The question is whether the person lives there by choice as a home, not whether a facility license covers the bed. If you are unsure about a building’s license, ask the county before you file as if it were a house.

- Own home is about living in an abode of one’s choosing, not about the deed. A rented apartment or a room in a relative’s house can count. A hospital, nursing facility, or licensed community care facility does not.
- If someone wants to stay home, is IHSS automatic?
- No. Medi-Cal must be determined, the Health Care Certification must reach the county, and a social worker must authorize tasks and hours. Wanting to stay home is the purpose of the program, not the approval.
- Does IHSS pay for someone to keep a parent company?
- Hours are tied to authorized hands-on or household tasks, and to other listed categories such as protective supervision when the county finds it is needed. Companionship by itself is usually a different idea.
