Using IHSS and Home Health at the Same Time

Two benefits, one home

Adult children often meet both programs after a hospital discharge. A home health nurse or therapist may visit for a skilled need. In-Home Supportive Services may later pay a family member or another provider for authorized personal care and household help. Those can exist together. They are not the same application and not the same notice.

IHSS builds on Medi-Cal and a county assessment. Home health builds on a clinician order and a home health authorization under Medi-Cal, Medicare, or another payer. Confusing them at the IHSS visit can hide personal care need, or it can sound like you are asking IHSS to become the nurse. What each program is for

IHSS, under WIC 12300 and MPP 30-757, may authorize personal care, domestic and related tasks, accompaniment to health-related appointments or alternative resource sites, protective supervision, paramedical services when ordered and authorized, limited heavy cleaning or yard hazard abatement, and teaching and demonstration. The social worker ranks function in the home. The Notice of Action is the IHSS list that counts.

Home health is skilled care in the home: nursing and therapy that licensed staff must do. It is not companionship. It is not ordinary landscaping. It is not routine childcare. It is also not a substitute for living in a facility. If your parent is still in a hospital or nursing facility, neither program is “the facility.” Plan the home they will return to, then file SOC 295 for that home.

How to keep the assessment from mixing the two

1. List morning and evening personal care: who bathes, dresses, and helps with toileting.
2. List skilled visits: which agency, which licensed role, which task they already do.
3. Bring SOC 873 or alternative documentation for IHSS, not the entire hospital dump unless asked.
4. Tell the social worker which tasks would remain if home health ended.
5. Tell the home health intake which tasks an IHSS provider already does or will do.

The last two sentences prevent double-counting and gaps. If home health is time-limited, the county still needs a picture of ongoing personal care. If a skilled task is moving to an IHSS paramedical request, say that you are asking about an order and county authorization, not assuming the nurse’s visit becomes IHSS automatically.

Paperwork stays in two stacks

IHSS wants SOC 295, SOC 873 or acceptable alternative documentation, and later provider enrollment if a family member will be paid. Extra hospital dumps can slow the IHSS file. Keep medical records for the county packet and send a page when the IHSS office names it. Home health has its own face sheet, orders, and agency consents. Do not put those agency pages in the IHSS envelope unless the county asked for that exact page.

Medi-Cal eligibility sits under IHSS. Home health may also need Medi-Cal or Medicare. If Medi-Cal is pending, both sides can stall. Check BenefitsCal or the Medi-Cal worker. Share of cost questions belong there. Do not write a guessed amount on either packet.

What neither program is

• Facility care in a hospital, nursing facility, or licensed community care facility
• Companionship visits as the only service
• Ordinary landscaping or general home repair
• Routine childcare
• A guarantee that an adult child will be paid before enrollment and authorization

Protective supervision remains an IHSS category for a mental impairment or mental illness when authorized. It is not a home health sitter. Accompaniment is IHSS when authorized to a health-related appointment or alternative resource site. Driving alone is usually a different need, and a medical transport benefit is yet another path.

When you are the family provider

You can help with authorized IHSS tasks and still greet the home health nurse at the door. You do not become the nurse. You do not cancel the nurse by enrolling as an IHSS provider. Finish orientation, SOC 426, SOC 846, and a background check where required before you expect the State check. Record IHSS time the way the county asks, including EVV when it applies. Do not record a skilled nursing visit as IHSS time unless the county authorized that exact task as IHSS.

If the county cannot discuss the IHSS file with you, ask for the local release. Keep copies of both the IHSS notice and the home health orders. When a task moves from one program to the other, you will want both papers in the same folder.
