IHSS Accompaniment to Health-Related Appointments

The category the statute actually names

WIC 12300 lists accompaniment to health-related appointments or alternative resource sites. The coverage page uses the same phrase: accompaniment during necessary travel to those places. MPP 30-757 keeps the worker inside listed categories. That is why this guide repeats the official wording instead of saying “IHSS transportation” as if every car trip counted.

Driving or transportation, except authorized accompaniment, is usually a different need. A medical transport benefit, a family driver, or another program may cover a ride. Ask the county how a trip is treated on this case. Do not invent a mileage rate or a wait-time rule.

What “going along” means at the assessment

Accompaniment is about the recipient needing someone with them for necessary travel to a health-related appointment or an alternative resource site. Describe who currently goes, why the person cannot complete that travel safely alone, and what happens at the destination that still needs the helper. Stay with a typical month, not a one-time emergency, unless that emergency is now the pattern.

If the helper’s only role is holding the steering wheel while the recipient could otherwise go alone, say that honestly. The worker may treat that as driving, not accompaniment. If the helper also provides personal care at the appointment (toileting, transfers), name that as personal care plus travel, and let the worker sort the lines.

Health-related appointments and alternative resource sites

Health-related appointments are the clinic, therapy, and similar visits the recipient already has. Alternative resource sites are the other program locations the county treats as that second official destination. Ask the social worker which sites they count. Do not expand the list to every errand you wish someone else would drive.

Food shopping and other authorized errands can appear as related services when the assessment supports them. That is household help connected to staying at home, not accompaniment. Heavy cleaning or yard hazard work is a different limited category. Mixing every trip into “accompaniment” hides the task the worker is supposed to rank.

How this sits next to home health and Medi-Cal

A home health nurse coming to the house is not accompaniment. IHSS and home health can still run in the same month. If the skilled visit is in the clinic instead of the house, accompaniment might be the IHSS question, while the clinic visit remains a medical benefit. Tell both programs who travels with the person.

Medi-Cal may have its own transportation processes. Ask Medi-Cal staff about those. Do not assume the IHSS notice replaced them, and do not assume they replaced IHSS accompaniment. DHCS Medi-Cal is the program page. The county offices apply both programs locally.

What accompaniment is not

• Driving or transportation as the only need, without authorized accompaniment
• Companionship outings that are not health-related appointments or alternative resource sites
• Ordinary landscaping runs or general errands the notice does not authorize
• Routine childcare or school transport
• Facility care, or travel that is really a hospital or nursing-facility admission
• Skilled nursing during the trip (that is a different benefit or, if it fits, ordered paramedical services)

Protective supervision can matter when a mental impairment or mental illness creates risk during travel, but it is still a separate category. Do not relabel companionship as accompaniment. Do not relabel a Sunday drive as a health-related appointment.

Forms and the notice

You still send SOC 295 and return SOC 873 or alternative documentation. Extra hospital appointment printouts can wait unless the county asked for a specific page. Keep those records for the county packet. After the visit, read whether accompaniment appears on the Notice of Action. Keep a copy.

If you will be the provider who goes along, finish enrollment before you expect payment for authorized time. Record time the way the county asks. Ask how they want accompaniment time shown when EVV applies. If the county cannot discuss the case with you, ask for the local release.
