Medi-Cal and share of cost

Medi-Cal Renewal and an Open IHSS Case

Why keeping Medi-Cal current matters for IHSS, and what to do if a renewal letter arrives, without inventing deadlines.

About 4 minutes to read. Based on CDSS and DHCS public program pages retrieved 2026-09-13. County offices apply these rules. Hourly rates, hour totals, and wait times are local and change.

Short answer

IHSS builds on a Medi-Cal eligibility determination. If Medi-Cal lapses at renewal, the IHSS foundation is gone and the home-care case can stall or stop. When a renewal letter or BenefitsCal request arrives, respond the way that notice asks. This guide does not invent a due date. The letter in your hand is the one that counts.

Who decides

County Medi-Cal staff decide the renewal. The county IHSS office still needs an active Medi-Cal determination to keep IHSS complete. BenefitsCal can carry the renewal paperwork. It does not replace the county decision.

What to do next

If a Medi-Cal renewal letter or BenefitsCal notice arrives, complete what it asks, keep a copy, and tell the IHSS worker if Medi-Cal status changes.

Renewal is how Medi-Cal stays the foundation

An open IHSS case can feel finished: Notice of Action in the drawer, provider enrolled, timesheets moving. Medi-Cal still has to stay current. CDSS lists a Medi-Cal eligibility determination among the IHSS criteria. If that determination lapses, IHSS does not keep a private exception for families who are tired of mail.

How the two programs work together is the same logic at the start of a case. Renewal is that logic a year later, or whenever Medi-Cal asks again. Adult children who only watch the IHSS worker miss the letter that actually matters.

What to do when a letter arrives

Read the notice. It will say what Medi-Cal wants and how to return it. Many parents also see a task on BenefitsCal. Using BenefitsCal covers helper access. Complete the pages. Upload or mail the proof they named. Keep a copy. Call Medi-Cal once if a question is unclear.

This page will not invent a number of days. Notices print their own dates. Blogs that say “always within N days” go stale and are wrong for the letter in your hand. If you cannot find the date, call the county Medi-Cal office and read the notice number aloud.

Tell IHSS when Medi-Cal status moves

If renewal is approved and nothing else changed, IHSS may already see it. If a share of cost appears, disappears, or changes, ask both offices. An unpaid share of cost can be deducted from the provider warrant. If Medi-Cal closes, call IHSS and ask what the file needs. Do not keep submitting timesheets and hoping.

  • Watch mail and BenefitsCal, not only the IHSS voicemail.
  • Respond to the renewal the way the notice asks.
  • Ask Medi-Cal whether share of cost changed.
  • Tell IHSS if Medi-Cal closes or the amount changes.
  • Ask again how you can be listed if workers will not speak with you.

Do not start over unless the county says so

A renewal is usually a continuation of an existing Medi-Cal case. Opening a second BenefitsCal application because the renewal screen was confusing can create a duplicate. If the parent moved counties, ask both counties which office owns Medi-Cal and which owns IHSS before you file anything new.

SOC 873 and the IHSS assessment are not the Medi-Cal renewal. Do not send the Health Care Certification to Medi-Cal unless they asked for a specific medical page. Do not send a renewal packet to the IHSS social worker and assume Medi-Cal received it. If the parent is in the hospital when the renewal arrives, still answer the Medi-Cal notice. A hospital stay is not own home for IHSS, and it is also not a reason to ignore Medi-Cal mail. Ask a social worker at the hospital, and the county, which address should receive the next notice.

If you are listed on the file, ask Medi-Cal to confirm the mailing address and the BenefitsCal login the parent actually uses. Renewals fail when the letter goes to an old apartment or an email no one checks. Fix that before the next cycle, not after a closing notice.

If the letter looks like a denial

Read it twice. Some notices are requests. Some are closings. The words on that page, and the county hearing information printed there, are the process. This site does not write a script or a timeline. Ask Medi-Cal what the notice means. Ask IHSS what happens to authorized hours if Medi-Cal is not current.

Official renewal doors

Use BenefitsCal, DHCS Medi-Cal, CDSS IHSS, and the county office list. If you want help lining a renewal letter up with an open IHSS file, you can ask for help.

Questions people ask

If IHSS is already approved, can we ignore Medi-Cal mail?
No. IHSS still needs Medi-Cal. A renewal that is not completed can undermine the case the social worker already authorized.
Where do I renew?
Follow the letter. Many families use BenefitsCal. Some renew with the county Medi-Cal office. Do not open a brand-new application if this is a renewal of an existing case unless the county told you to.
Will IHSS send the same deadline?
Not always. Medi-Cal and IHSS are related files. Watch the Medi-Cal letter and BenefitsCal, not only the IHSS worker.

Ask for help navigating care for a parent

Share a few details if you want help organizing IHSS paperwork so a parent can stay home. This form is for follow-up contact, not an eligibility decision. Your county IHSS office and Medi-Cal decide the case.

  • A coordinator follows up
  • County decides eligibility
  • You can stop anytime
  • Educational help

For example: starting an IHSS application, provider enrollment, or county forms. Keep medical records and diagnoses for the county packet.
