Can an Adult Child Be Paid as an IHSS Provider in California?

Payment follows after county orientation, provider forms (SOC 426 and SOC 846), a Department of Justice background check where required, and authorized hours on the parent’s case. Adult children are commonly allowed after enrollment
If you are an adult son or daughter already helping a parent stay home, In-Home Supportive Services is often the program people mean when they say Medi-Cal can pay a family caregiver. The California Department of Social Services says IHSS providers may include family members, friends, neighbors, or someone from a county Public Authority list. An adult child of an adult recipient sits in that first group. The county does not treat you as a special exception. It treats you as an individual provider who still has to finish enrollment before the State can issue a check.
Two tracks have to finish. The parent’s IHSS case has to authorize tasks and hours after Medi-Cal, the application, the Health Care Certification, and the in-home assessment. Your provider file has to finish orientation and the enrollment forms. When pay starts is the meeting point of those tracks, not the date you first helped in the kitchen.
What “paid as a provider” actually means
The IHSS recipient is the employer of the individual provider. Your parent (or their authorized representative) hires you, trains you on how they want the work done, supervises the day-to-day help, and can end the arrangement. The State issues the provider payment for authorized time after timesheets are accepted. Hourly rates vary by county because unions negotiate with the local employer of record. Ask the county for the current rate. Do not plan around a number you saw on a forum.
You must be authorized to work in the United States to enroll. You must be able to give the authorized hands-on or household care safely. Companionship by itself is not what the social worker authorizes. The Notice of Action is the list of tasks and hours that can be claimed.

- Does an adult child need a special waiver to be the provider?
- CDSS treats family members, including adult children, as ordinary individual providers after enrollment. The extra payability limits that apply to a spouse or a parent of a minor do not apply the same way to an adult child of an adult recipient.
- What enrollment steps come before a paycheck?
- County provider orientation, the SOC 426 enrollment form, the SOC 846 provider agreement, a DOJ background check where required, and setup for recording authorized time. Ask the Public Authority which session and packet they use.
- Can more than one adult child share the work?
- The recipient is the employer and can hire more than one enrolled individual provider for authorized hours. Each person completes their own enrollment. The Notice of Action still sets the hours that can be claimed.
- What if I already live with my parent?
