IHSS vs Private Home Care in California

Updated September 16, 2026. Reviewed September 16, 2026 against public CDSS, DHCS, Medicare.gov, VA, and CareScout 2025 California tables. County wages, hours, and wait times still change. Official .gov pages decide.

Side-by-side: California IHSS (Medi-Cal, county notice) versus a licensed private home care organization. When each fits, who pays, and how to run both without mixing timesheets.

Based on public CDSS, DHCS, Medicare.gov, VA, and California Department of Insurance pages retrieved 2026-09-16. County, Medi-Cal, Medicare, VA, and insurer staff decide what actually pays. Hourly rates, hour totals, waiver slots, and wait times are local and change.

Short answer

IHSS is California’s Medi-Cal in-home personal care program. A county notice authorizes tasks. An enrolled individual provider, often a family member, is generally the worker. Private home care is a licensed home care organization you hire at a private rate. They solve different jobs. Some households use both. Keep the county timesheet and the agency invoice on separate calendars.

Who decides

County IHSS and Medi-Cal staff decide the public plan. CDSS Home Care Services oversees licensed organizations. Each agency sets private terms. You decide whom to hire for extra hours.

When IHSS usually fits

  • The person lives in California in a home they choose and may meet Medi-Cal rules.
  • A family member or friend can enroll as the provider after the county authorizes tasks.
  • The need is ongoing personal care and related household help, not a short skilled-nursing episode.
  • You can wait on county process for the authorized list while other help covers the gap, if needed.

When private home care usually fits

  • You need coverage before a notice exists, or more hours than the notice authorizes.
  • No family member can be the provider, and you want an organization with backup staff.
  • You need evening or weekend patterns the public plan does not list.
  • You want a written private contract you can end without a county change of provider.

Running both without mixing the books

IHSS timesheets follow the notice. Agency invoices follow the contract. If both people are in the house on the same morning, write down who did which task. The county can ask. The agency can ask. Mixing them is how families lose a warrant or pay twice for one hour. How to choose a home care agency has the license questions. How to apply for IHSS is the public sequence.

Planning money for the private row belongs on compare California care costs, which uses CareScout 2025 statewide medians. That matrix is not an IHSS wage. Ask CDSS and the county for the local public rate. Ask each agency for its private rate.

Soft next step

If you want help lining up the county packet, ask for help. California Home Help explains the path. The county still decides IHSS. An agency you hire still decides whether it can staff the shift.

Questions people ask

Does IHSS send an agency?
Usually no. Recipients generally hire an individual provider. A Public Authority list can help you find one. That is not the same as contracting a home care organization.
If we can pay privately, should we skip IHSS?
Not automatically. IHSS can cover authorized tasks after the county decides. Private pay can fill gaps. Ask both desks how benefits interact. Do not assume they cancel.
Can an adult child be the IHSS provider and still hire an agency?
Sometimes, for extra hours the notice does not cover. Do not timesheet the same hour twice. The recipient remains the IHSS employer.
Where do wages and private rates live?
IHSS wages: CDSS county wage page and the local office. Private rates: each licensed organization. CareScout statewide medians on /costs are planning estimates only.

See if IHSS may apply

A coordinator can walk through Medi-Cal, county forms, and the home visit with you. The county still decides eligibility and hours.

For example: starting an IHSS application, provider enrollment, or county forms. Keep medical records and diagnoses for the county packet.
