The county process, in order

1. Confirm the person lives in California in a home they choose. File in that county, not the hospital’s county unless that is also the home.
2. Start or confirm Medi-Cal on BenefitsCal or at the county Medi-Cal office. IHSS is built on that determination.
3. Download SOC 295 from the CDSS forms catalog and submit it to the IHSS office on the CDSS county list.
4. Return SOC 873 or the alternative the county names. A licensed health professional signs the usual certification.
5. Prepare a typical-day list for the in-home assessment. Ask to be listed on the case if you need the county to speak with you.
6. Read the Notice of Action. If you will be the provider, finish enrollment after the county authorizes tasks.

Medi-Cal is not a side errand

IHSS does not replace Medi-Cal and Medi-Cal does not replace the IHSS visit. DHCS runs Medi-Cal. CDSS runs IHSS. The same county building may house both desks. They are still two files. You describe bathing, dressing, grooming, toileting, transferring, feeding, and related household tasks tied to staying home. Protective supervision is a separate category when mental impairment creates a risk of injury. Do not treat a diagnosis as an hour total. If a family member will be paid, that person still enrolls as an individual provider after authorization: orientation, provider forms, and a background check when the county requires it.
