Updated September 16, 2026. Reviewed September 16, 2026 against public CDSS, DHCS, Medicare.gov, VA, and CareScout 2025 California tables. County wages, hours, and wait times still change. Official .gov pages decide.
Compare home care and assisted living in California: IHSS own-home rules, licensed RCFE, Medi-Cal, and the Assisted Living Waiver. Counties decide IHSS. CareScout medians are planning only.
Based on public CDSS, DHCS, Medicare.gov, VA, and California Department of Insurance pages retrieved 2026-09-16. County, Medi-Cal, Medicare, VA, and insurer staff decide what actually pays. Hourly rates, hour totals, waiver slots, and wait times are local and change.
Short answer
In California, home care means help inside a house or apartment the person already lives in: IHSS, a licensed Home Care Organization, or both. Assisted living is usually a licensed residential care facility for the elderly. IHSS is an own-home program. A licensed community care facility is not that home. The Assisted Living Waiver is a separate DHCS path at participating sites. You and the person decide whether to move. County IHSS, Community Care Licensing, and DHCS each decide their own piece.
Who decides
You and the person decide whether to move. Community Care Licensing oversees residential communities. County IHSS and Medi-Cal apply only while the person lives in a qualifying home. DHCS and the waiver agency decide Assisted Living Waiver slots.

California home with help versus licensed assisted living
| Question | Home with help in California | Licensed RCFE |
|---|---|---|
| The place | House or apartment the person already lives in | Licensed residential care facility for the elderly |
| Public program that often applies | IHSS after a county assessment, plus Medi-Cal | Assisted Living Waiver at participating sites; not IHSS as own-home care |
| Who authorizes personal care | County Notice of Action and, if hired, a private agency contract | The community’s license, base fee, and care add-ons |
| Who pays room and board | The household rent or mortgage, as today | Usually private pay unless a waiver or other program applies |
| When it often fits | The house can be made safe and someone can cover the hard hours | The house is no longer safe, or 24-hour presence cannot be staffed at home |
Why California families need a California comparison
The shorter home care versus assisted living page is the setting fork. This page adds the California public-program layer: Medi-Cal, In-Home Supportive Services, licensed Home Care Organizations, licensed residential care facilities for the elderly, and the Assisted Living Waiver. Those names are not interchangeable. Mixing them is how families lose an IHSS file or sign a community contract they cannot unwind.
CDSS says IHSS is for a person living at home or in an abode of their own choosing. Acute-care hospitals, long-term care facilities, and licensed community care facilities are not that home. DHCS administers Medi-Cal and the Assisted Living Waiver. Medicare.gov is clear that Medicare generally does not pay long-term custodial care or assisted living rent. Cite those pages. This site does not invent a wait list or a county wage.
When staying home with California help usually fits
- The person wants to remain in that house and the layout can be made safer.
- Medi-Cal is active or in process, and IHSS may cover authorized personal care after a county visit.
- A family member can enroll as an IHSS provider, or a licensed Home Care Organization can fill gaps.
- Nights are coverable by family, authorized IHSS, private shifts, or a mix.
Start the public path with how to apply for IHSS in California. File in the county of the home address. IHSS by county and the CDSS office list are the directory. Bay Area families comparing several counties can use aging parents, home care, and IHSS in the Bay Area.
When a licensed California community usually fits
- The house cannot be made safe even with extra aides.
- The person needs on-site staff through the night and you cannot staff that pattern at home.
- Meals, medication reminders, and housekeeping in one licensed setting would reduce missed care.
- You have read when home care is not enough and still land on a community.
Ask each community whether it is a licensed RCFE, what is in the base fee, and whether it participates in the Assisted Living Waiver. Slots, wait lists, and room-and-board rules are local. DHCS decides. This page will not invent them. The Long-Term Care Ombudsman is the public complaint path if a resident already lives there.
Money without invented IHSS wages
Cost of home care and cost of assisted living use CareScout California statewide medians as planning estimates. They are not community quotes and they are not IHSS wages. For the public hourly figure, use find your county IHSS wage, which links only the official CDSS table. Pay answers whether Medicare or Medi-Cal typically pays for each setting.
If you want help organizing public paperwork while you tour, ask for help. Signing a community contract is still your decision. For companion education about staying home, Sevah publishes stay home. That page is not a county filing and does not diagnose.
Questions people ask
- Does IHSS follow a parent into a California assisted living community?
- Usually no. IHSS is for a home of the person’s choosing, not a licensed community care facility. Ask the county before you sign a move-in.
- Does Medi-Cal pay assisted living rent in California?
- Not as ordinary IHSS. The Assisted Living Waiver is a DHCS program at participating sites. Slots and room-and-board rules are local. DHCS decides.
- Is the CareScout figure an IHSS wage or a community quote?
- Neither. CareScout California statewide medians on the cost pages are private-pay planning estimates. IHSS wages live only on the official CDSS wage page.
- Can we try home care first in a Bay Area or inland county?
- Many families do. File IHSS in the county of the home address. A San Francisco job does not create a San Francisco file for a parent in Stockton. Set a review date if nights or wandering are already unsafe.
