Short answer

Home care means help inside a house or apartment the person already lives in: IHSS, a licensed home care organization, or both. Assisted living is a licensed residential community with room, board, and on-site help. IHSS is for own home. It generally does not follow someone into that community bed. Home care often fits when the house can be made safer. A community often fits when nights or wandering already exceed what the house can staff. Cost medians for each setting are on the cost hub.

Who decides

You and the person decide whether to move. Community Care Licensing oversees residential communities. County IHSS applies only while the person lives in a qualifying home. DHCS and the waiver agency decide Assisted Living Waiver slots.

When assisted living usually fits

The house cannot be made safe even with extra aides.
The person needs on-site staff through the night and you cannot staff that pattern at home.
Meals, medication reminders, and housekeeping in one licensed setting would reduce missed care.
You have read when home care is not enough and still land on a community.
