What Typically Happens at an IHSS Reassessment
A periodic IHSS reassessment looks at need again. The county schedules the visit. A new notice may change authorized tasks up or down.

IHSS cases are reassessed periodically. A county social worker looks at need again, often using a typical-day conversation in the home. A new Notice of Action may keep, raise, or lower authorized tasks. The county schedules the visit. This page does not invent how often that happens.
Who decides
Your county IHSS office schedules the reassessment and authorizes any new plan. Medi-Cal eligibility still has to be in place. Hearing language, if you disagree with a new notice, is printed on that notice.

Need is not frozen on the first notice
The first in-home assessment produces a Notice of Action. Life at home continues. Counties reassess IHSS need on a periodic schedule they control. Families call this an annual reassessment because that is how it often feels. This page will not invent a statewide interval. Your worker can tell you what is typical locally. Put the appointment letter in the same folder as the last notice.
A reassessment is still an assessment. The worker uses the same kinds of tools: function in this home, a typical day, and the service categories the program can authorize. Preparing for the home visit still applies. So does what to say: actual help, not a bravest day, not an inflated day.
What the visit usually looks like
- The county schedules a time in the home, or another method they use locally.
- They confirm who lives there and who gives help.
- They walk through daily tasks: bathing, dressing, meals, mobility, household work.
- They may ask what changed since the last notice.
- They may ask for an updated Health Care Certification if the file needs it.
If memory has changed, describe function: missed medications, stove risk, wandering. See memory changes and the assessment. Do not coach a category. The worker decides what to authorize. If you want to sit in, your parent should want you there, and you should already be listed for follow-up calls.
The new notice can move in either direction
A reassessment is not a raise. Function might be more independent in one task and need more help in another. The new notice is the plan that counts for timesheets. Provider enrollment usually stays in place through a reassessment. A new notice does not, by itself, enroll a sibling. If who gives the care is also changing, that is a provider request alongside the visit.
Medi-Cal still has to be alive under the case
IHSS sits on Medi-Cal. A renewal or a share-of-cost change can arrive in the same season as a reassessment. They are different units. Ask Medi-Cal about coverage and share of cost. Ask IHSS about tasks. BenefitsCal is often the coverage door. Do not assume a completed IHSS visit repairs a lapsed Medi-Cal file.
If the scheduled visit is not enough because something just changed
A fall, a hospital discharge, or a sudden need for more hands-on help can land between periodic reviews. That is when you ask for a new assessment after a change rather than waiting for whatever letter usually arrives. Tell the worker you are reporting a change in need, not asking them to skip their tools.
Keep the typical-day notes honest
A task calendar can help you remember a usual week. Honest notes only. Do not build a performance week. Do not shrink the help to look independent if that is not the typical day. If a sibling will sit in, agree on one typical-day story so the worker does not hear three versions of breakfast.
Provider pay continues on the current notice until a new one arrives
Keep recording authorized time on the notice you already have. A scheduled reassessment is not a pause button. If enrollment for a new helper is still open, finish that packet on its own track. - Is there one statewide month when every case is reviewed?
- No. Timing is local. The county schedules. Do not copy an interval from a friend. Ask your worker what is typical for this file.
- Will hours automatically go up because a year passed?
- No. The worker looks at current function. Hours can stay the same, go up, or go down. The new notice is the list that counts.
- Is a reassessment the same as asking after a fall?

- Should I bring a diagnosis stack?
- Bring what the county asks for. A typical-day list is usually more useful than a chart dump. Keep SOC 873 current if they say the medical piece is stale.
