What Happens When an IHSS Packet Is Incomplete

An incomplete IHSS packet sits or comes back. The usual gaps are a missing signature on SOC 295, a blank or unsigned SOC 873, and a name that does not match the Medi-Cal case. Extra hospital records do not usually fix those gaps. Complete the pages the county asked for, then confirm the file is whole.

What “incomplete” usually means
County IHSS offices look for a usable SOC 295 and a completed SOC 873 or acceptable alternative documentation. Incomplete packets sit. Missing signatures, a blank SOC 873, or a name that does not match the Medi-Cal case can send the file back. Fill what is asked. Extra medical records do not usually help.
This is the sticking point listed on the families page. It is also the reason what to send prefers complete pages over a hospital dump. A thick envelope can still be incomplete if the required lines are empty.
Missing signatures
SOC 295 needs the signatures the form asks for. An adult child cannot silently replace the applicant on those lines. If your parent can sign, they sign the applicant lines. If they cannot, ask the county what they accept. Do not leave the line blank and hope a cover letter is enough.
Provider forms are a second signature problem. SOC 426 and SOC 846 are signed by the person who will be the provider. Mixing those unsigned pages into the recipient packet does not complete SOC 295.
